As a Social Worker in our hospice and home health programs, you will have the opportunity to provide essential support and guidance to patients and their loved ones during a difficult time. Daily Responsibilities: Conduct psychosocial assessments to identify patient and family needs Connect patients and families with community resources for additional support Provide supportive counseling and advocacy for clients Collaborate with interdisciplinary teams to develop comprehensive care plans Offer grief and loss counseling to individuals and families Required Skills: Experience in medical social work or hospice care Strong understanding of biopsychosocial assessment techniques Ability to provide client advocacy and supportive counseling Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) Knowledge of community resources and services Nice to Have Skills: Experience in Home Health and Hospice Family counseling and bereavement counseling expertise Anger management and mental health counseling skills Company Information: 1st Care Palliative and Hospice Services PA is dedicated to providing compassionate end-of-life care to patients and families in the Allentown area.
