Job Description

Professional Case Management is seeking a dedicated Physical Therapist to provide high-quality, in-home rehabilitation services to clients with complex medical needs. In this role, you will travel to patients’ homes to evaluate functional status, develop individualized treatment plans, and deliver evidence-based physical therapy interventions that promote mobility, safety, and independence. You will work primarily with former energy workers and their families, many of whom present with chronic, work-related, and multi-system conditions requiring thoughtful, patient-centered care. As a Physical Therapist with Professional Case Management, you will conduct comprehensive assessments, set measurable goals in collaboration with patients and caregivers, and regularly reassess progress to adjust plans of care. You will educate patients and families on home exercise programs, safe body mechanics, use of assistive devices, fall prevention, and strategies to improve quality of life within the home environment. You will coordinate closely with nurses, physicians, case managers, and other members of the healthcare team to ensure integrated, holistic care. The position requires strong clinical judgment, excellent documentation skills, and comfort working autonomously in the field while remaining closely connected to a supportive interdisciplinary team. You will complete timely and accurate electronic documentation, uphold all regulatory and payer requirements, and contribute to care conferences and case reviews.
