Professional Case Management is seeking a dedicated Physical Therapist to provide high-quality, in-home rehabilitation services to clients with complex medical needs. In this role, you will travel to patients’ homes to evaluate functional status, develop individualized treatment plans, and deliver evidence-based physical therapy interventions that promote mobility, safety, and independence.
